Enrollment Procedures

Dear Parent/Guardian and Future Lenicia B. Weemes Students,  

Welcome to Lenicia B. Weemes!  We are currently accepting applications for grades ETK - 5th, and we hope you will decide to join our community.  We want to provide you with as much information and guidance as possible to make this process smooth and efficient.  Below is some information to help guide you through the process:

REQUIREMENTS FOR KINDER, TK, ETK ENROLLMENT

• Kinder students must be 5 years old on or before September 1 of present year
• Transitional Kinder (TK) must be 5 between September 2 through December 2
• Expanded Transitional Kinder (ETK) must be 5 years between December 3 and June 30

HOW TO ENROLL FOR ALL GRADE LEVELS

OPTION 1:  Working directly with Weemes Elementary School to Enroll for 2019-2020 or 2020-2021 School Year

• Please review the ENROLLMENT CHECKLIST.  (Spanish ENROLLMENT CHECKLIST). 
• As indicated in the ENROLLMENT CHECKLIST you will need the following:
  • Completed Student Enrollment Form (Spanish Form)
  • Completed Student Emergency Form (Spanish Form)
  • Completed Student Housing Questionnaire (Spanish Questionnaire)
  • Proof of Residence*
  • Proof of Student's Age*
  • Identity Verification of Parent/Guardian/Educational Rights Holder/Caregiver ("parent")*

  • Proof of Immunization*

• Print the 3 completed forms (Student Enrollment Form, Student Emergency Form, and Student Housing Questionnaire) and collect the 4 documents (Proof of Residence, Proof of Student's Age, Identity    Verification, Proof of Immunization).  
• Once you have printed the completed forms and have the documents, contact us for an appointment:

• If you have trouble accessing and printing the forms contact us via email, phone or text at above numbers/email for assistance.
• If you are having trouble locating any of the 4 required documents contact us via email, phone or text at above listed numbers/email for guidance.
• To access the Checklist and Forms mentioned above click on the links which are blue and underlined.  The forms and checklist are also available on the left, go to "Enrollment Forms in English."  They are also available in Spanish, "Formas de Ingreso en Español."

OPTION 2:  Working with LAUSD to enroll for CURRENT year ONLY.  Cannot enroll for 2020-2021via this option

• Go to the LAUSD Enrollment Website (Please note the instructions below are also on the LAUSD Enrollment Website, so if you go to that website you do not need to reference this page.)
• Please review the ENROLLMENT CHECKLIST.  (Spanish ENROLLMENT CHECKLIST). 
• As indicated in the ENROLLMENT CHECKLIST you will need the following:
  • Completed Student Enrollment Form (Spanish Form)
  • Completed Student Emergency Form (Spanish Form)
  • Completed Student Housing Questionnaire (Spanish Questionnaire)
  • Proof of Residence*
  • Proof of Student's Age*
  • Identity Verification of Parent/Guardian/Educational Rights Holder/Caregiver ("parent")*

  • Proof of Immunization*

• Upload all documents in the Parent Portal.  If you do not have an account, you can create one via the Parent Portal Login Screen.
• Once the LAUSD Enrollment Team reviews the documents, they will contact Weemes ES and the school will contact you for an appointment to complete your child’s enrollment process.  During the appointment you will be asked to provide photo identification, sign enrollment related forms, and may be giving additional forms and materials to support virtual/remote instruction
